GP Initiative
Since 2004, we have been encouraging carers to register with their GP and have developed a carer's registration form for them to use to record their details, as well as a poster for them to display promoting carers registration.
We send out this information annually to GP Surgeries in Hampshire. The latest edition of the Letter to GP Surgeries, Poster and GP Registration form are below in PDf format.
